If you or someone in your household uses electrically-powered medical equipment, request to be added to our medical conditions list.
Download and print our medical equipment request form (English, Spanish).
The form requires a physician's statement and signature. You must complete a form each year while using Dominion Energy, and we must receive it within 30 days of the anniversary date of the last form you sent. Otherwise, we'll assume the condition no longer exists and the medical notation on your account will be removed.
Please Note: Send all medical forms to demedical@domenergy.com.
If your power goes out:
- We will make every effort to resolve service quickly.
- Prepare yourself beforehand by having battery or manually-powered medical equipment available to you, if possible.
- Have a plan in place to receive proper care from someone else, perhaps a family or a friend, until your service is restored.
